In 2002, we founded Greenfield Care Center of Southgate, a 99-bed certified Medicare and Medicaid facility, with the belief that we will continuously strive to grow and provide the best quality care for the residents of its community. Our professional staff is committed to providing residents with an individualized plan of care designed to meet each individual’s physical, social, spiritual and emotional needs. It is our interdisciplinary team approach to quality care that allows us to assess and create a care plan tailored to meet the individual goals of our resident and their family.
We accept Medicare, Medicaid, Managed Care Insurance, Worker’s Compensation and Private Pay.
