The primary purpose of your job position is to plan, organize, develop and direct the overall of our Nursing Service Department in accordance with current federal, state and local standards, guidelines and regulations that govern our facility and as may be directed by the administrator and the Medical Director, to ensure that the highest degree of quality care is maintained at all times.

Essential Responsibilities

  1. Maintain an acceptable standard of nursing practice and professional decorum.
  2. Be courteous, considerate and cooperative when communicating with all facility staff, residents and public.
  3. Promote and maintain good public relations on behalf of the facility.
  4. Knowledge of Federal and State Regulations governing long term care nursing facilities.
  5. Knowledge and implementation of the facility’s policies and procedures.
  6. Direct and assist Licensed Nurses in clinical performances areas.
  7. Perform function and responsibilities of a Charge Nurse as assigned.
  8. RN: Perform functions and responsibilities of Assistant Director of Nursing Services on the day shift as directed.
  9. React appropriately to emergencies and disaster situations.
  10. Notify Administrato and/ or Director of Nursing Services of any serious situation and requiring administrative direction.
  11. Perform other duies as may be assigned by the Director of Nursing Services.
  12. Other duties and responsibilities may be assigned by the administrator due to facility needs.

Here’s what you need to succeed:

  • Nursing degree from accredited college or graduation from approved RN program required
  • Must possess current, unencumbered license to practice as an RN in California
  • Current valid CPR certification required

Job Description

As the weekend NOC Licensed Vocational Nurse at Greenfield Care Center of South Gate, you will provide direct nursing care that includes and is not limited to medication administration, phlebotomy, and documenting all nursing related assessments in the EMR. You will be part of a multi-disciplinary team to support the needs of Greenfield Care Center of South Gate clients.

Responsibilities

  • Conducts daily physical and mental status assessments and documents them accordingly
  • Participates in client observations to promote abstinence of behaviors and safety
  • Organizes, coordinates, and follows up on all consults with physicians / medical personnel for tests, etc. and provides the appropriate diagnosis for laboratory, radiology and other diagnostic testing
  • Creates and maintains weight and vital signs in EMR and checks daily for accuracy. Rechecks any documented weight or vital sign that is unusual and documents
  • Completes all Alsana Nursing Assessments, Care-plans, S.O.A.P Notes, Admission Note, Discharge Notes, Noc Shift note and accurately documents in the medical record. All LVN Notes to be co-signed by the Nurse Manager and/or RN
  • Administers first aid according to facility policy, including all appropriate follow-up documentation
  • Obtains insurance updates to get precertification for medications and other coverage
  • Perform all other duties as assigned
